Membership

- Applicants must be at least 18 years old but not more than sixty (60) years old as of the enrollment date.

 

- He or she must be a regular or not regular employee of any organized group, company institutions or churches accredited by CCT Group of Ministries (http://www.cct.org.ph)

 

- In good health condition

 

MEMBER’S LEGAL DEPENDENTS

 

1. Legal dependents of a married member are as follows:

 

1.1 Member’s legal spouse;

1.2 At most two (2) of the member’s biological and/or legally adopted children, single, and fourteen (14) days old but not more than twenty-one (21) years old; and

 

1.3 Any of the member’s biological and/or legally adopted children over twenty-one (21) years old, single, has a congenital disability, incapacitated to work and chiefly dependent on support.

 

1.4 If the husband and wife will enroll separately in BLIP, then they are both considered individual “principals” and as such entitled to have two (2) dependent children on their coverage.

 

2. Legal dependents of a single and unmarried member without biological children are his/her biological parents who are sixty-four (64) years old and below.

 

3. Legal dependents of a single and unmarried member but with children are as follows:

 

3.1 At most two (2) of the member’s biological and/or legally adopted children, single, and fourteen (14) days old but not more than twenty-one (21) years old; and

 

3.2 Any of the member’s biological and/or legally adopted children over twenty-one (21) years old, single, has a congenital disability and incapacitated to work.
 

In this case, biological parents of those single (unmarried) members but with children will no longer be considered as their legal dependents.

 

MULTIPLE COVERAGE

 

- If an insured is covered under more than one Certificate of Membership, either as the principal member or as legal dependent under some other’s Certificate of Membership, the amount of indemnity upon loss shall be based on whichever Certificate of Membership has the higher Amount of Benefits.

 

GRACE PERIOD

 

- Any member who fails to pay his/her weekly contribution shall be given a grace period of forty-five (45)calendar days from the date of default of contribution payment within which to remit the contributions.

 

2. In case of death or total and permanent disability (whichever is applicable) of the member or his/her legal dependent during the grace period, arrears shall automatically be deducted from the benefits payable.

 

TERMINATION

 

1. The insurance coverage under the Certificate of Membership of BLIP shall automatically terminate under the following conditions whichever comes first:

 

1.1 upon death or total and permanent disability of the member;

1.2 upon resignation from any groups affiliated with CCT MBA;

1.3 upon resignation of the member from CCT Mutual Benefit Association Inc;

1.4 upon attainment of age sixty- five( 65) of the member;

1.5 upon expiration of the grace period if no payment is received by then; and

1.6 upon termination of membership with cause by CCT MBA.

1.7 upon payment of the member’s Equity Value.

 

2. The legal dependent’s coverage is co-terminus with the member’s coverage. Termination of coverage shall be without prejudice to any claim arising prior to such termination.

BLIP Benefits

1. Death Benefit

 

1.1 Natural Death

 

1.1.1 Upon DEATH OF THE MEMBER, due to natural causes, the designated beneficiary/ies shall be indemnified in accordance with CCT MBA Table of Benefits as shown above.

 

1.1.2 Upon death OF MEMBER’S LEGAL DEPENDENT, due to natural causes the member shall be indemnified in accordance with CCT MBA Table of Benefits.

 

It shall be understood that upon death of the member due to natural causes his/her Certificate of Membership is automatically terminated.

 

1.2 Accidental Death

 

1.2.1 Upon DEATH OF A MEMBER due to accident, the designated beneficiary/ies

shall be indemnified in accordance with the CCT MBA Table of Benefits.

 

1.2.2 Upon DEATH OF THE LEGAL DEPENDENT due to accident, the member shall be indemnified in accordance with CCT MBA Table of Benefits. It shall be understood that upon death of the member due to accident his/her Certificate of Membership is automatically terminated.

 

2. Total and Permanent Disability (TPD)

 

2.1 Upon TOTAL AND PERMANENT DISABILITY OF THE MEMBER, s/he shall be indemnified in accordance with the CCT MBA Table of Benefits.

It is understood that upon total and permanent disability of a member, his/her Certificate of Membership is automatically terminated. Hence, once the total and permanent disability benefit has been paid, no more benefits are payable in case of the member’s subsequent death.

 

2.2 Upon TOTAL AND PERMANENT DISABILITY OF MEMBER’S LEGAL SPOUSE, the member shall be indemnified in accordance with the CCT MBA Table of Benefits.

It is understood that once the total and permanent disability benefit for the member’s legal spouse has been paid, then no more benefits are payable in case of his/her subsequent death.

 

3. Member’s Equity Value

 

A member shall be entitled to an Equity Value equal to at least fifty per centum (50%) of his/her total gross life insurance contributions paid. Interest shall be credited to the Equity Value annually at a rate to be determined by the Board of Trustees but in no case less than the prevailing savings account interest rate of the top three (3) commercial banks. This Equity Value, inclusive of interest thereon, is payable upon termination of his/her membership from CCT MBA including, but not limited to, death or total and permanent disability. Upon reaching the termination age of sixty-five (65) years old, the member shall be entitled to payment of the member’s Equity Value.

up-to-date BLIP (Micro-insurance) contributions.

SIGURADO BENEFITS

Pre - existing Condition

1. CCT MBA is not liable for any loss occurring during the first year of coverage of the  Member under this endorsement or within one (1) year from the date of last reinstatement of this coverage, except if caused
by accident or diseases acquired within that period as certified by a competent physician,
other than TWO THOUSAND PESOS (Php 2,000.00).

2. Pre-existing Condition is a condition for which the Member received medical treatment or  has consulted
a physician for medical treatment or has been taking medications during the twelve (12) months prior to the effective date of coverage under this endorsement or twelve (12) months prior to the date of its last reinstatement.


Suicide

CCT MBA shall be liable if the Member commits suicide after one (1) year from the effective  date of coverage of this endorsement or date of its last reinstatement. Suicide committed in the state of insanity will be compensable regardless of the date of commission.

Grace Period

A Member shall be given a grace period of forty-five (45) calendar days from the date the premium is due within which to remit the premium. In case of death of the Member during the grace period, arrears shall automatically be deducted from the benefits payable.


Termination

​

The coverage under this endorsement shall automatically terminate on the earliest of the 
following:

1. Date of termination of membership in CCT MBA:
2. Date the Member reaches his sixty-fifth (65th) birthday; 
3. End of the grace period if premiums remain unpaid during the period.


Termination of the optional benefits shall not defeat any claim arising before such termination.
Once terminated, further payments or acceptance of any premium shall not create any liability under these optional benefits other than to refund the same. 

Reinstatement


1. Coverage under this endorsement that has terminated may be reinstated provided the 
Member is in good health as certified by a competent physician. The reinstatement shall 
take effect upon payment of the premiums in arrears.

2. Reinstatement can only be performed within a period of one (1) year from the date the 
coverage under this endorsement terminates.
